New Hire Documents
In your offer letter, you were asked to complete all new-hire documents before you attend Children’s Mercy Orientation (CMO), on your first day of employment. These documents are required to finalize your employment.
Be on the look-out for separate emails from:
- service@validityscreening.com – This email will include instructions to complete section one of your I-9, which is a federally required form used to verify your identity and legal authorization to work in the United States.
- noreply@axionhealth.com (subject line “Children’s Mercy Occupational Health Clinic”) – This email will include instructions to set-up your account with our Children’s Mercy Occupational Health system. You will also be asked to complete a few surveys and upload your vaccination records.
