Questionnaires used at CM to assess asthma control
Test for Respiratory and Asthma Control in Kids (TRACK)
Test for Respiratory and Asthma Control in Kids (TRACK) for under 5 years of age AND has a history of 2 or more episodes of wheezing, shortness of breath, cough lasting more than 24 hours AND previously prescribed bronchodilator medications OR diagnosed with asthma. A score of less than 80 indicates asthma may not be well controlled.
Childhood Asthma Control Test (C-ACT) for ages 4-11 years old
A score of 19 or less indicates asthma may not be well-ontrolled.
Asthma Control Test (ACT) for ages 12 years and older
A score of 19 or less indicates asthma is not be well controlled.
